AUTO-RENEWING MEMBERSHIPS

• The Modo Limited and Unlimited Memberships are auto-renewing every 4 weeks.
• The payment will be charged to your card on file every 28 days.
• No minimum commitment.
• We require a 7-day cancellation notice before your recurring payment if you want to cancel your membership by completing this form.
• Accepted payment methods: VISA, Visa-Debit or MasterCard.
• Members can suspend their memberships for 30, 60 or 90 days each year free of charge. 
• Additional suspensions in a calendar year are subject to a $25 administrative fee.

LATE CANCEL FEES

• Drop-ins and class cards are forfeited if class cancellation is less than 4 hours before class.
• Any membership holder who cancels a class less than 4 hours beforehand will be charged a $12 late-cancellation fee.  
• If you have not cancelled your class and are on a membership, a No-Show fee of $20 will be charged to your card on file. 
• Workshop fees will be forfeited for cancellations less than 24 hours.

FOR ALL PASSES

• All memberships, drop-ins, and passes are non-refundable and non-transferable.
• Class cards are valid for 12 months after the date of purchase.
• GST is applied to all memberships and passes.
